Maybe someone said “Put your hands behind your back.” While he/she put their thumb on the lens of the camera.
The gentleman’s cap is called a newsboy cap or a driving cap. This photo is probably from the late 1920’s to early 1930’s.

That portrait of the unexpected thumb can be quite the bummer when it showed up in those old film photos. At least in this case, you could still see the subjects of the picture. It appears composition was not this photographer's forte, however.
